I did this layout quite a while ago, but we were searching the scrapbooks when my son asked how long ago that it was that he broke his arm! I thought that this was pretty clever~it's made of gauze and molding paste, then painted and sanded. Then, we used Sharpie to write on it just as we did the cast! I actually "chiseled: the letters on to the metal tags with a set that MM made back then. It took me forever to make!!
