I got inspired to do this by a project on the QKD blog (http://darkinksorority.blogspot.com/2011/02/pieces-tealight-holder-card.html) which referenced this tutorial http://whiffofjoy.blogspot.com/2010/10/tealight-gift-card-by-jane-johnson.html).
I didn't make mine the same as either of theirs, but with some basic math it's pretty easily adaptable to customizing.
